Obverse: A bow, symbolizing a birthday present and the words "HAPPY BIRTHDAY" in Hebrew and English.
Reverse: The words "till 120", the traditional Jewish birthday wish, in Hebrew and English. In the bronze medal, the inscription is surrounded by concentric circles of numbers from 1 to 120. The silver and gold medals have a bouquet of flowers to the right of the inscription.
Edge: Bronze/tombac and silver medals - State emblem with the words "STATE OF ISRAEL" in Hebrew on the right and in English on the left, with serial number. Bronze/tombac medal - "BRONZE" in Hebrew and English; Silver medal - "SILVER 935" in Hebrew and English; Gold medal - milled.
Serialization: Bronze/tombac and silver medals are numbered.
